I am currently only taking self-pay clients and I offer a sliding-scale to make care more accessible.

Sliding Scale Program

GMH offers several sliding-fee scale positions to income-eligible uninsured or underinsured clients based on annual household income for our Individual therapy services.

Income guidelines and acceptable verification for the sliding-fee scale are listed below.

Extenuating circumstances regarding ability to pay e.g., high medical bills, etc. will be taken into account when agreeing upon a fee.

At Going Mental Health it has always been a goal to improve access to mental health services for everyone.

A waitlist to utilize the sliding fee scale is available for all cash clients being seen at GMH. If you earn less than $80,000 per year in combined family income, you may benefit from applying for our sliding-scale fee program below.

I understand that this information is confidential. I agree to inform the business office of any  change in these circumstances so that a re-evaluation can be made.

The sliding fee scale is based on the average family of 2-4 people and can be adjusted higher or lower  based on the number of people living in the home. If any changes arise in income please notify GMH as-soon-as possible so adjustments can be made to the fee.

All sliding fees are based on a regular 45-50 minute session. Sliding fee clients pay the same rate for an intake session as for a regular session.

Sliding fee scale is available for all cash clients being seen at GMH. Fees are as of January 1, 2023 and are adjusted periodically.

Qualifying clients shall provide proof of their annual income, unemployment, or disability payments for all immediate family members living in your household. Gross income is all income from all sources before taxes. Applicants should provide a copy of either two consecutive pay stubs, or the previous year’s income tax return, W-2 and/or a 1099 Form.
Please describe any unusual expenses or circumstances affecting your financial situation. I understand that this information is confidential. I agree to inform the business office of any change in these circumstances so that a re-evaluation can be made.

If a third party is paying for the services of a client, an applicable discount rate will be based on the income of the person paying for the services, not the client's income.

Approval for a sliding-scale discount should be applied prior to commencing services. Existing clients who apply for this discount and are approved will receive the discount on future services. The discount is not retroactive.

A limited number of client spaces are available with this discount. If a space is not available at the time of approval, you may be placed on the waiting list for the next available discounted space. Alternatively, you may choose to begin services at the regular cost, but if you decide to take advantage of the discount when a space opens, you may have to change providers.

If Going Mental Health determines that a client has submitted false information to qualify for this discount, the discount will be reversed and the additional services fee will be due immediately. GMH reserves the right to change the terms, or cancel this discount program, at any time.
